As we enter cold and flu season, I wanted to take a moment to remind you about the Chieftain Health Clinic! Our goal is to help keep you, your children, and the students and staff of Logan School District healthy and learning. We want healthcare to be easy to access for our school community, and we are here to help!
 FLU SHOTS ARE AVAILABLE NOW!
We currently have flu vaccines in stock! If you or your child need a flu shot this season, we would be happy to get you scheduled.
You do not have to be a regular patient of our clinic to receive a flu shot! We welcome students, family members, and school staff, even if you see another provider for your regular healthcare.

WHO CAN BE SEEN AT THE CLINIC?
The Chieftain Health Clinic is available to:
- Students of Logan School District
- Family members of students
- Logan School District faculty and staff
We see patients ages 18 months and older.
WHAT SERVICES DO WE OFFER?
We provide a variety of healthcare services for the whole family, including:
- Physicals: Sports physicals, work physicals, and yearly wellness exams.
- Vaccinations: Routine vaccines and flu shots.
- Sick Visits: Care for common illnesses such as strep throat, colds, flu, ear infections, and other everyday illnesses.
- Ongoing Health Conditions: Help managing conditions such as asthma, ADHD, anxiety, depression, high blood pressure, and diabetes.

If you or your family are looking for a new primary care provider, the Chieftain Health Clinic may be a great fit! We provide care for everyday illnesses, routine checkups, and ongoing health conditions. We would love the opportunity to care for you and your family!
WHERE ARE WE LOCATED?
The clinic is attached to Logan High School. We also offer transportation to the clinic for students attending the middle and elementary schools. Transportation must be arranged in advance, so please call us before your child’s appointment if transportation is needed.
We understand that taking time off work to bring your child to an appointment isn’t always easy. With the proper parent or guardian consent on file, students may be seen at the clinic during the school day without a parent having to leave work to attend the appointment. This allows students to receive the care they need while making things a little easier for busy families!
INSURANCE AND CONSENT FORMS
We accept most major insurance plans, including Medicaid. If you have questions about insurance or need help completing the necessary consent forms, we are happy to walk you through the process.
We also offer electronic consent forms through MyChart, making it easier to get started!

Visits are by appointment. Whether you need a flu shot, a physical, care for an illness, or a new primary care provider, we are here to help!
